The motto of our hospice is ”IT'S ABOUT LIVING.” Our mission is to provide you with the medications, medical equipment, counseling, and innovative symptom management in addition to assistance with comfort, general support, personal care, encouragement, and relief for caregivers. We pride ourselves on being an innovative hospice and as our motto says, we want to help you live every day to the fullest possible extent.
The team includes a general internist along with a cardiologist who assists when the needs arise. A Family Nurse practitioner also assists with house calls and as your healthcare provider in the home. Eight specialty physicians are consulted on an as need basis to improve the patients quality of life. Every two weeks every patient is reviewed with the entire hospice team. A pharmacist reviews all medications along with recommendations to assist with symptom management. As a teaching hospice, students from local colleges complete visits with our hospice staff.
Our inter-disciplinary team assists patients and their families to meet the goals of care. We provide symptom management, psychological, and spiritual support for terminally ill patients.
